Incident Report

To report an incident of intolerance complete the Incident Report Form. The Incident Report can either be emailed as an attachment to president@tctj.org or mailed to:

Unless you INDICATE OTHERWISE, it is assumed that the contents of this report can be used to substiantiate frequency and types of incidents of intolerance.  We will not use the information except as indicated by you in the "incident followup information" section, or otherwise by your permission.
